Job description

This position is identified for an Aboriginal and/or Torres Strait Islander person and is intended to constitute a special measure under section 8 (1) of the Racial Discrimination Act 1975 (Cth), and section 57 of the Anti-Discrimination Act 1996 (NT).

Who are we?

Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region in the Northern Territory.

We work to address the social determinants of health including the broader issues that affect our people's health, through evidence-based research and public advocacy. We also work to empower individuals, families, and communities to optimise their health.

ABOUT THE ROLE

The Family Partnership Worker (FPW) works with the Nurse Home Visitor (NHV) works and provides a comprehensive home visiting service to pregnant women with an Aboriginal and/or Torres Strait Islander baby and their families. The FPW works with the NHV to foster a culturally safe, therapeutic relationship with pregnant women and promote high level engagement and successful program outcomes.

WHAT YOU WILL DO

  • Provide culturally safe home visits to women who are eligible for the ANFPP and their families and works with ANFPP team members to enhance the team's capacity to remain culturally safe
  • Demonstrate responsibility for own development through participation in all learning opportunities.
  • Effectively communicate with a multidisciplinary team including clients and external service providers to ensure external stakeholder services to clients.
  • Conduct all activities in a manner consistent with national health accreditation standards, DDHS Occupational (Work) Health and Safety Policy and procedures.
  • Ensure all documentation, records management and the health information system database are maintained and completed in accordance with established standards, policies, and protocols to ensure continuity of care.

SELECTION CRITERIA

Essential

  • Demonstrated experience working in a clinical practice or community health education program.
  • Communication and interpersonal skills with the ability to interact with Aboriginal people from diverse backgrounds as well as with people from a wide range of cultural and social backgrounds.
  • Awareness of and sensitivity to Aboriginal culture and history; and an understanding of social, health and wellbeing issues affecting Aboriginal and Torres Strait Islander families.
  • Ability to work independently, and as part of a team, with an ability to plan, manage and prioritise tasks and resources to deliver outcomes within time and budget considerations.
  • Ability to develop, coordinate and facilitate group and community-based activities in a timely and effective manner.
  • Ability to acquire family and social information to assist with Family Support needs and an understanding of confidentiality.
  • Demonstrated written skills in the preparation of detailed reports, general correspondence, and documentations, as well as demonstrated computer skills with the ability to enter information into client history notes; and,
  • Preparedness to undertake and complete all ANFPP training and other workplace training as required with a willingness to travel interstate for training.

Desirable

  • Experience in working in an Aboriginal Community controlled  Health Service or similar complex and high risk environment
  • Certificate qualification in Community Services or equivalent

EMPLOYMENT IS SUBJECT TO

  • Willing to undergo a Police Check.
  • Ability and willingness to obtain a Working with Children Clearance (Ochre Card).
  • Current driver's licence
  • Current CPR certificate or the preparedness to gain one within 6 months of commencement.
  • Fully vaccinated against COVID-19 or an exemption as listed on the Australian Immunisation Register (AIR) be submitted.
  • Full rights to work in Australia.
