Who are we?

Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region in the Northern Territory.

We work to address the social determinants of health including the broader issues that affect our people?s health, through evidence-based research and public advocacy. We also work to empower individuals, families and communities to optimise their health.

About the role

The Family Support Officer (FSW) is responsible for maintaining high level standards of community practice and foster acceptance of the Danggal Dalby (Strong Kids) team in the community. The FSW will work closely with the Danggal Dalby team to support and coordinate the provision of program within our DDHS clinical services.  The role will support families to navigate child health and paediatric appointments for assessment/diagnosis and therapy. The FSW is an identified position.

Responsibilities:

  • Maintain regular contact with clients within the scope of practice that have been identified by the Danggal Dalby Team.
  • Facilitate client referrals to specialist and other allied health services, both internal and external to DDHS, and when required act as the liaison person on client?s behalf.
  • Provide a range of health education messages, information and advice to clients and families about healthy lifestyles choices and the importance of child health checks.
  • Undertake relevant assessments to identify client support needs, gaps in service delivery and potential service provision enhancement and report on such activities as necessary.
  • Assist with planning, delivery, and evaluation of specific health program and community activities.
  • Participate in regular meetings with the team to record health program report, and client?s feedback regarding the delivery of programs and services provided.
  • Assist with client follow up and recalls as directed and coordinated by the Danggal Dalby Team Leader.

To be successful you will need:

Essential

  • Demonstrated ability to work within and maintain strict confidentiality guidelines and policies.
  • Demonstrated communication and interpersonal skills with the ability to liaise with people from a wide range of cultural and social backgrounds to both obtain and convey information relevant to their care.
  • The ability to interact with a range of health professionals including medical practitioners and allied health professionals.
  • Ability to work independently, and as part of a team, with an ability to plan, manage and prioritise tasks and resources to deliver outcomes within time and budget considerations.
  • Demonstrated computer skills especially in the use of client information systems or similar.

Desirable

  • Demonstrated experience in neurodevelopment screening (ASQ TRAK) for children and youth
  • Demonstrated experience and competency in working with Communicare.
  • Relevant experience/certificates in Child and adolescent health
  • Experience in working in an Aboriginal Community Controlled Health Service or similar complex environment such as service industry or public administration.

  • 6 weeks annual leave per annum.
  • Salary packaging of up to $15,990 per annum.
