Position Overview The focus and purpose of this role is to provide person-centred support to assist TLC clients reach their full potential, enhancing their quality of life, through programs focused on the individual.
Duties and Responsibilities include but are not limited to:
- Establish and maintain network and partnerships with community groups, local business, schools and other service providers
- Liaise and work cooperatively with a range of stakeholders including families, group homes, case managers, allied health specialists and specialised health units both in the public and private hospitals.
- Ensure that plans are reflective of an individual’s strengths, gifts and abilities
- Ensure programs are developed and reviewed to reflect client goals
- Actively contribute in the formulation of department objectives, goals and strategies
- Provide high-quality group-based activities for NDIS participants
- Ensure the continuing improvement of services delivered by participating in quality improvement processes to ensure best practice
- Relevant experience supporting people with an intellectual disability and/ or children and young people in a related area/ field.
- Certificate IV or equivalent in Community Services, Mental Health, or related field
- Demonstrated experience in facilitating group based social and basic living skill activities for adults with a lived experience of severe and complex mental health problems.
- Ability to engage with participants with strong group facilitation skills
- Understanding of the recovery model and how this applies to adults experiencing severe and complex mental health problems
- Unrestricted Driver’s License
