The role
In this role you will support Community Gateway's purpose to make Health and care simple for optimal living. With technology at the forefront you will be a driving force in designing and implementing outcome focused in-home and online Health and Wellbeing programs. You will be responsible for:
Quality and Risk Management
Management of the clinical governance framework
Leading a team of Nurses to provide high-quality person-centred care
Educating clients, carers and team members to promote good health
Facilitating the development and execution of clinical support plans
Managing clinical care referrals and onboarding of NDIS, HCP and private client's
Lead and participate in continuous professional development and incorporate new skills in client care
As the Team Leader Gateway Wellbeing Services, you will play a pivotal role in participating in the overall development and implementation of Community Gateway's strategic plan.
About you:
Essential:
Relevant qualifications and current Nursing Registration with AHPRA accreditation
Previous experience of building new services and achieving KPI’s
A sound working knowledge of clinical governance frameworks
Proven track record of redesigning services to improve quality, ensure relevancy for clients and maximise efficiencies
Proven experience of developing restorative and reablement programs
Demonstrated examples of improving the client experience through innovative practices
A track record of developing a defining culture and undertaking creative activities to imbed it within the organisation
Ability to develop high performing and engaged teams and implement quality processes
A current Working with Children Check
A current NDIS worker screening check
Proof of COVID 19 Vaccination or exemption
This role will require travel to our offices in Wollongong, Nowra and Kempsey.
Desirable:
Experience in working with a wide range of service providers
Experience in working for a community-based Board of Management
Awareness and understanding of the CHSP Guidelines, Disability Service Standards and Community Care Standards for Aged Care
