We are seeking enthusiastic Health Professionals (Nurses, Psychologist, Speech Pathologist, Occupational Therapist, Physiotherapist, Social Worker) to join our team in this diverse and challenging role. No two days are the same, as you assess and manage complex cases within our Aged Care Assessment Team (ACAT). This role will test you, but you’ll be provided with structured training, supervision and opportunities to develop specialist core and advanced practice skills in a secure and supported work environment.

This is a Recruitment Pool for Permanent Full Time, Permanent Part Time, Fixed Term Full Time, Fixed Term Part Time and Casual appointments. Fixed Term and Casual appointments will be made with the possibility of extension(s) and/or permanency.
Suitable applicants will be placed in a pool from which appointments may be made when similar vacancies occur. Appointments may be made from this pool until 31/12/2025.
About this role
ACAT is a Commonwealth funded program that funds NMHS and other organisations to provide a team of nursing and allied Health Professionals to clinically assess the care needs of older people who have complex care needs. ACATs help older people and their carers to access appropriate levels of support such as Home Care Packages and residential care.
The Health Professional ACAT role(s) are unique in that they interact with older people and their carers in the community. The Health Professional ACAT role provides the opportunity to assess, educate and support older people and their carers in planning for the older person’s care. Older people are generally assessed in their own homes and NMHS ACATs cover most of the northern suburbs, so confidence in driving in the community is essential.
