Care & Connects is an innovative NDIS provider who is dedicated to delivering the best quality support to each of our participants. To assist with this, we have a dedicated Psychosocial Disability program led by experienced Mental Health professionals, delivering Supported Independent Living (SIL) and Community Participation (CP) supports across the Adelaide region.

About you:

We are looking for compassionate individuals with a genuine desire for supporting individuals with specific conditions and needs. You possess a strong understanding of the NDIS framework and are dedicated to its principles.

Qualifications

• Minimum Certificate III / IV in Disability / Mental Health or equivalent.
• Medication Administration Accreditation
• NDIS Worker Screening
• Manual Handling Training
• First Aid and CPR
• CPI/MAPA/Mental Health First Aid is desirable
• Any additional trainings such as PEG/Bowel/Diabetes is an advantage
• 100 Points ID

Selection Criteria

• Availability to work in the following locations and times:
  • SA 5108: 12:00 pm to 14:30 pm and 18:30 pm to 23:00 pm
  • SA 5107: 6:00 am to 2pm, 1:00 pm to 5:00 pm and 5:00 pm to 10:00pm plus passive night.
  • SA 5068: 1:00 am to 6:00 am , 8:00 am to 4:00 pm and 6:00 pm to 11:00 pm
• Minimum 1 year in High Care and/or Mental Health support, including but not limited to bipolar disorder, anxiety, schizophrenia, intellectual disability, challenging behaviours, high needs care, etc.
• An understanding of the impact chronic pain, trauma, grief and loss has on an individual and their families, particularly those with complex needs.

Your tasks may include but not be limited to the following:

• Providing personalized care and support to NDIS participants, including personal hygiene; meal preparation; medication; and activities of daily living.
• Implementing positive behaviour support plans and crisis intervention strategies
• Provide emotional and practical support to participants according to their individual needs.
• Monitor participant’s progress and report any changes or concerns as required
• Maintain accurate records and documentation in line with NDIS and organisational requirements
