We have an exciting opportunity for experienced and motivated Case Manager to join our Home Care Services team on a temporary full time basis. You will be working to provide a high quality service of managing and maintaining a growing client base by assisting each person individually to attain their goals and maximise their independence and abilities to meet their everyday needs.
Your responsibilities may include:
- Draw on knowledge and experience of working with frail aged persons and/or in dementia care;
- Work flexibly and reliably within a multidisciplinary team within a case management capacity;
- Practice a client centred approach and respond constructively to client’s choices and preferences, maintaining dignity and respect for their wishes;
- Manage a case load of clients overseeing successful implementation of care plans, HCP budget monitoring and necessary documentation;
- Review clients and carry out reassessment on a regular basis as clients care needs change, following level upgrade, hospital discharge and at a minimum annually.
- Plan, monitor and evaluate delivery of service as per client’s goals and preferences;
- Engage in business development and support the growth and marketing of Private Home Care;
- Support fellow Case Managers, administration and rostering needs if necessary.
Essential Criteria:
- Have experience in Aged Care or a clinical background
- Be committed to Health Care, St Luke’s Values and St Luke’s Code Of Conduct;
- Be committed to ongoing educational and professional development;
- Be an energetic and enthusiastic team player and contributor;
- Successfully manage client case load, their budgets and their roster needs.
- Well-developed communication and interpersonal skills, particularly building healthy rapport with clients, carers and colleagues;
- Be proficient in office computer systems and be willing to share knowledge where necessary;
- Maintain a current driver’s licence with access to a car with comprehensive insurance;
Desired Criteria:
- Tertiary qualification in Allied Health, Social Work or Registered Nursing related discipline
- Demonstrate experience working in aged care
