AddressWe are seeking an experienced Care Manager to work with our Aged Care, disability and Attendant Care clients in Sydney.
As a Care Manager, you will primarily support Aged Care clients with a government Home Care package, NDIS funding and insurance funding for personal injuries (icare). Your role will include participating in client intake and care planning in partnership with a Registered Nurse, case management and coordinating and monitoring all aspects of service to ensure clients' needs are met.

What is required?
- Case Management experience in supporting clients with government Home Care Packages (required), NDIS funding, and insurance clients receiving Attendant Care (icare)
- Tertiary qualifications in Aged Care, Disability, Social Work or equivalent
- Superior interpersonal and written/verbal communication skills
- Exceptional customer service skills
- Ability to build strong relationships and networks within the community
- A current National Police Check
- NDIS Worker Clearance
The salary package is negotiable and will include the opportunity to earn a performance-based bonus.

Our team of support workers are required to complete a comprehensive recruitment process and are selected based on their understanding, compassion, and experience in providing quality care services. We then match each carer with clients based on personality, skill set, and experience. We continue to educate and support our carers with ongoing education and training, all overseen by our experienced nurses.
