AddressStart Nursing Services is looking for a suitable candidate to work as “Home Care Coordinator
Assistant” for 2 days with the possibility to be increased to 3 days in our office at Bella Vista
(NSW). Your working hours is from 0900-1700.
Essential criteria’s:
- To be able to speak Persian / Farsi fluently in addition to English
- To have degree or diploma related to community or home care services or nursing
- To have at least 1-2 years’ experience working in community, home care or nursing sector
-Clear understanding of Aged Care & Home Care sector, relevant standards and consumer directed care
- To have a clear understanding of My Aged Care portal & accessing clients and processing referrals
-To have clear understanding of PRODA & Aged Care Provider Portal and claiming process
- Strong interpersonal skills with an understanding of diversity when working with clients and colleagues
- Demonstrated ability to develop positive relationships with a range of clients & their families
High level of computer literacy
- Understanding of clinical governance and continuous improvement
- Possess a 'can-do' positive attitude and work well in a team environment
- To have an outgoing personality with high energy levels
- Well-developed communication skills (verbal and written) with excellent telephone manner
- The ability to prepare reports and correspondence in clear and concise language
- Ability to solve problems and think quickly under pressure
- Ability to multitask with a strong attention to detail while keeping calm and focused
- Excellent networking, consultation skills to develop new referral sources and relationships with appropriate external organizations
- To have valid and cleared police check, working with children check, NDISWC & First Aid
- To have a full driving licence and reliable car
