We are looking for a Home Care Coordinator who, as part of a multi-disciplinary team, will provide comprehensive Community Care Programs within the parameters and criteria of the Eyre and Far North Local Health Network (EFNLHN).
In this role you will identify needs, perform assessments, implement, coordinate, maintain and evaluate and review the home and community-based ageing and disability programs supporting those who wish to remain living in their own home with some assistance.
Eligibility
Applicants must have:
- experience in a case management role,
- experience working in a multi-disciplinary team,
- experience working with individuals, families, groups and wider communities across a range of culturally diverse backgrounds and with complex health care needs,
- some understanding of cultural differences, particularly related to Aboriginal people, primary health care principles, health resources and services and quality improvement processes and practices.
