Home Care Coordinator

is a family-owned aged care provider operating 36 residential aged care homes, with in-home and community care programs in WA and NSW. We believe that our commitment to providing high quality care can make a real difference to the lives of our consumers and their families.
We are seeking a compassionate Home Care Coordinator to join our team at on a full-time basis.
Reporting to our Home Care Manager, you will work alongside an established team to ensure that all home-care clients receive services of the highest quality, all the while providing them and their families with respectful and efficient customer service.
Your well-honed clinical, communication and problem-solving skills are demonstrated while coordinating Home Care packages and working as part of an inter-disciplinary team, as well as when providing relationship-centred care that ensures the dignity and respect of care recipients.

• qualification as Enrolled or Registered Nurse with AHPRA registration
• a NDIS Worker Screening Check
• evidence of Influenza vaccination
• full working rights in Australia
