Cockburn Care is a well-established and highly respected HACC, NDIS and Home Care Package provider. The organisation is offering a great opportunity for a highly organised Home Support Services Coordinator to help lead the CHSP Home Support team.
The Position
The position requires high level organisational skills, supervision skills and team leadership to meet the needs of diverse range of frail aged people living in the community, including people living with dementia, Aboriginal people, people who are LGBTQ&I and people from a CaLD background.
You will need a clear understanding of CHSP and HCP guidelines, principles of Wellness and Enablement and the Aged Care Quality Standards.
You will need a broad skill set including excellent customer service, capacity to schedule, excellent team work and supervisory skills, good documentation skills and experience using client service management software (such as Sandwai)
The Home Support Services Coordinator will be based at 6 Ingram St Hamilton Hill.
The successful applicant will possess:
Relevant qualification and experience in the administration of Aged, Community and/or Health Care Services.
Experience using client service management software.
Developed computer skills and computer literacy
Well-developed understanding and knowledge of the support of frail aged people from diverse backgrounds.
Well-developed supervisory skills including supportive attitude towards team members.
Excellent interpersonal, written and verbal communication skills.
A knowledge and understanding of Aboriginal People and their culture.
Understanding of budgets and ensure adherence to the same.
Salary: $54.94 - $58.28 per hour (inclusive of casual loading) depending on qualifications and experience.
