Your new companyYour new company is a leading community services provider that runs a variety of important social programs across the region to support vulnerable people within our local community. They have an opportunity to add a Homelessness Case Manager to their team in Mt Isa.
Your new role
Provide case management support tailored to the program's target group, implementing early intervention strategies to engage participants and achieve program and individual goals.
Conduct initial assessments to verify program eligibility. Continuously assess progress, documenting participants' achievements and barriers in support plans and reviews.
Utilise electronic data systems to securely record and manage participants' personal information in alignment with service agreements, legislation, and program policies.
Foster relevant community partnerships through networking, actively referring and promoting the organisations' programs.
Attend meetings, engage in professional supervision, and participate in ongoing development.
Undertake outreach and community engagement activities as needed.
Contribute to program development and implementation, including planning and providing community education and support.
What you'll need to succeed
Degree in Social Work, Human Services, or Behavioural Sciences, OR a Cert 4/ Diploma in the community sector, with relevant experience working within a case management framework
Eligibility to work in Australia.
A valid Driver's Licence.
Current or eligibility for both a Disability Worker Screening and a Working with Children Blue Card.
