About the role

This role will work in conjunction with the clinical co-ordinator to assist in continuing acute inpatient care within The community. The role will function as a sole practitioner and is required to provide safe and competent care for acute care in patients' homes.

The role will be required to problem solve any issues that arise, provide an expert clinical service relevant to standards and guidelines and initiate referrals within The multidisciplinary team where required.

Staff will be required to work a rotating roster (AM and PM shift) 7 days per week. HITH rostering is based on a fair and equitable roster for all staff.

About you

To be successful in this role you should possess:

• Current AHPRA registration
• Minimum of 5 years acute medical/surgical post registration nursing experience or nursing specialty equivalent e.g. ICU/ED/CCU/ Oncology/Renal
• Highly developed clinical skills
• Highly developed interpersonal and problem-solving experience
• Demonstrated capacity to work autonomously
• Ability to work as part of a diverse team
• Strong interest in community care
• Well-developed problem solving skills
