About the Role
The House Leaders role is to ensure that the participant(s) needs, and best interest are facilitated during the care of Connecting Families Disability Services. This includes keeping and ensuring that the household meets legislative and Connecting Families standards including support worker training, completing all documentation and compliance for the household and vehicles associated with them.
- Will have the best understanding of the participant
- Training all new support workers that provide care in the house
- On-going training and support with the support workers
- Work with work force planning in weekly rostering
- If there is unfilled shifts or last minute cancellations you are expected to cover the shift
- Create and update routines in the household and ensure it is being implemented - Create and update meal plans in the household and display them
- Understand and follow the Restrictive Practices if there are any for the participant
- Attend important appointments e.g. Doctors appointment
- Attend fortnightly Town Hall meeting with the Team Leaders to discuss any concerns or updates for the participant(s)
- Keeping the house and participant’s clothes clean and hygienic
- Weekly Check List - Purchasing the groceries weekly for the household
- Planning activities and excursions
- Complete weekly environmental checklist for the household and provide them to Team Leaders weekly
- Complete SIL reports on a weekly basis
- Ensuring that there is enough mediation weekly for the participant(s)
- Ensuring the onsite folder charts are completed: o ABC Charts o Sleeping Chart o Bowel movement chart o Medication Chart o Third Party registers o Incidents o Food and Fluid Chart o Seizure Chart
Requirements
To apply for this role, you must have or be willing to obtain:
- Current Working with Children Check
- NDIS Worker Clearance and Blue Card
- Current police check conducted within the last 6 months
- Valid Australian driver's license (with a reliable and comprehensively insured car)
- Current first aid and CPR certificates

Salary
- 70,000 - 75,000
