Footprints Community is a well-regarded not-for-profit provider of community-based services, working in the community for over 30 years. We specialise in working with older people, those that experience disability, mental illness, as well as those who are at risk of homelessness.
The Toowoomba Housing Hub is an integrated service model, bringing together a range of community and government human service providers in one location to deliver coordinated, person-centred housing and support services to enable people to obtain and sustain appropriate housing.
The primary purpose of the Hub is to assist people with multiple needs to access collaborative housing, homelessness and human services, and community resources that are responsive to their housing goals and improve their lives. The Footprints team works with local agencies to access clients and coordinate assistance for their immediate housing needs and longer-term housing goals and aspirations.
Local Housing Service Centre (HSC) staff will co-locate with the Footprints team to deliver the Hub services with a focus on strong sector collaboration and high local recognition to assist people who present or are connected to the Hub with housing instability/homelessness combined with complex needs, particulary First Nations people. The team will provide client assessment and referral, case management, care coordination, mobile support and brokerage.
Purpose of the Role:
Reporting to the Team Leader- Toowoomba Housing Hub, the purpose of the Hub Case Worker is to assist clients who are experiencing housing instability, at risk of homelessness or homeless to access appropriate and sustainable housing, as well as community care and other support services, specifically targeted at avoiding homelessness or reducing the impact of homelessness. Working alongside individuals to support the process and application of practical tasks to apply for housing and or make appropriate referrals to other services.
This position will provide mobile support in the community from the Toowoomba Housing Hub.
About You:
- Relevant Certificate IV or Diploma (e.g. Community Services, Social Work, Human Services) or significant progress towards such a qualification, and/or demonstrated experience in the provision of community-based support services.
- Demonstrated knowledge and experience in working with a diverse client group including older people, people with disabilities, people experiencing mental illness and people experiencing homelessness or those at risk of, and people with disability and their carers/families.
- Empathy and understanding of the needs of older women, especially those with a lived experience of homelessness or housing stress.
- Demonstrated knowledge and/or experience in the delivery of trauma-informed practice.
- Demonstrated experience in community networking and working collaboratively with other service providers to plan and provide consistent, responsive, and appropriate client focused services.
- Well-developed written and verbal communication skills and computer literacy.
- Current class C driver license.
- National NDIS Worker Screening Check or current Yellow Card.
- Current First Aid Certificate (desirable)
