Central Coast Local Health District (CCLHD) are currently seeking a dedicated Social Worker to join the warm and supportive team at the Geriatric Rehabilitation Unit located at Wyong Hospital

About You

  •        You will require Tertiary qualifications in Social Work, with eligibility for membership of the Australian Association of Social Workers (AASW) (overseas qualifications must supply the AASW Overseas Assessment number). A demonstrated understanding of the current AASW Practice Standards as they relate to Inpatient Social Work. 
  •        A well-developed understanding the psychosocial issues that arise for patients and their families during chronic and acute illness and trauma.   Preferably demonstrated social work experience in the clinical specialty area where applicable.  
  • Demonstrated experience in providing support, assessment, counselling, and other intervention strategies to enhance social functioning. These may include individual and joint interviews, case conferences, practical assistance, referral to community agencies etc.

This role in Geriatric Rehabilitation centres on assessment, counselling, and referral for patients, clients, and their families or carers. Its primary aim is to address instances where psychosocial and environmental factors might influence the diagnosis, treatment, and recovery processes within the health service.
