About the role:
This is a phone-based role located working for a large NFP Aged Care Service Provider. Based in Burwood East, you will be the vital link between the consumer and implementation and review of Government funded services including home maintenance program that support the consumer to remain living independently and with dignity within their own home.
Responsibilities:
- Intake of new clients
- Reviews
- Assessment / Care Planning
- Service delivery
Monitor and manage referrals received through the My Aged Care Assessor portal
Liaise with older people referred through for an assessment, arrange appropriate services /referrals to ensure best outcomes for clients. Undertake Support Plan Reviews- identifying any changes required to support the consumers independence.
About you:
Experienced with intake of new clients, reviews, assessment, and care planning.
- Customer service focused
- Excellent phone manner
- Strong communication skills
- Ability to manage a high volume of calls and meet KPI's
- MAC portal and knowledge of the Commonwealth Home Support Program (CHSP) adn HCP (Home Care Packages)
