About the role

As an Intensive Family Support Case Manager at Better Together Community Support, you will play a pivotal role in supporting caregivers, children, and families to navigate challenges and achieve improved safety, wellbeing, and resilience.

Under the guidance of the Case Management Coordinator, you will utilize evidence-based social work principles within a holistic case management framework to provide tailored support services, advocacy, referral and case management to our families. The case management team works across 3 different programs, and although this role predominantly focusses on supporting families through the Intensive Family support program, there is an expectation that the successful candidate may also be able to offer support in the programs of Tertiary Family support and Youth Alcohol and Other Drug Care Coordination.

This role requires a deep understanding of the primacy of child safety, program activity, operational policy, the Human Services quality framework and other relevant legislation. This comprehensive understanding is vital to guarantee the provision of top-tier services that adhere to industry standards and prioritize the wellbeing and safety of children and families

Position responsibilities
  • Follow professional, evidence-based case management processes including; intake and assessment processes; developing and reviewing case plans in collaboration with family members.
  • Provide regular outreach and home-based support, offering foundational counselling, psycho-social education and coordination of services across various areas such as parenting, domestic and family violence, substance abuse, and mental / emotional health.
  • Apply an ethical framework alongside evidence-based social work principles to effectively support families, children, and the community, thereby enhancing the safety and overall wellbeing of families.
  • Collaborate with external organizations and internal teams to achieve the best outcomes for clients.
  • Monitor individual KPIs and program requirements set out by funding bodies.
  • Maintain accurate records and reports in accordance with organizational guidelines and procedures.
  • Demonstrate proficiency in understanding and utilizing computer database systems to facilitate efficient and precise record-keeping procedures.

  • Adhere to established work practices and support Better Together programs and procedures, and organisational goals.
  • Engage in collaborative efforts with colleagues, external agencies, and clients, ensuring alignment with organizational policies and procedures throughout the process.
  • Set priorities, plan, and organize your work effectively within established guidelines.
Reporting and Support
  • Provide timely and accurate reports on all aspects of responsibilities, contributing to team reports when required
  • Adhere to Records Management policy and procedure by keeping professional case notes and preparing reports according to guidelines.
  • Offer back-up support in the workplace as required to maintain services to clients and the general public.

SELECTION CRITERIA
Qualifications
  • Bachelor’s Degree in Social Work or equivalent.
  • Driver’s License.
  • Capacity to acquire a Blue Card.
Knowledge and Skills
  • Demonstrate proficiency in current principles and methodologies of social work and case management, reflecting a deep understanding of contemporary practices in the field.
  • Possess a comprehensive understanding of the complex social issues and barriers related to child protection, family dynamics, domestic violence, and metal/ emotion al wellbeing, enabling informed and empathetic support within these sensitive areas.
  • Strong relationship and rapport-building skills.
  • Competency in office software and database management.
  • Willingness to develop a comprehensive knowledge of organisational policies, procedures, practices, and contract requirements with an appreciation of Better Together’s vision, mission, and strategic priorities.
  • Cross-cultural awareness, fostering an inclusive approach that respects and embraces diverse perspectives and backgrounds.
Demonstrated experience in:
  • Proven experience in managing priorities and performance within a professional team.
  • Excellent communication skills, both oral and written, including proficiency in computer program, spreadsheet and database use.
  • Demonstrated experience in case planning, implementation and management.
  • Ability to engage effectively with the public and other organizations.
