About the role

As a member of the Case Management Team, you will work under general direction from the Case Management Coordinator to provide social work and case management services to achieve results in line with Better Together Community Support and program goals. 

The Case Management Team works across different programs. The primary programs include Tertiary Family Support Service, Intensive Family Support and the Youth, Alcohol and Other Drugs Case Management service. You will be expected to work as needed across these services and other appropriate Better Together programs to meet the needs of clients and Better Together organisational outcomes. 

Your work at this level requires a sound knowledge of program activity, operational policy, the Human Service Quality Framework, the primacy of child safety, and other relevant legislation, and service aspects of the work performed within your team.

Position responsibilities

  • Establishing case plans for clients, including the initial screening, intake, and assessment procedures.
  • Create and maintain the client’s case management plan including conducting assessments, and determining the services needed in collaboration with the client.
  • Provide assessments and interventions in the community including home visits, counselling, and coordination of services, focusing on the psycho-social needs of the client.
  • Assess case plan effectiveness, amending as necessary and anticipating current and forthcoming needs.
  • Provide referrals as needed and follow-up co-ordination as required.
  • Collaborate with other teams across Better Together to achieve the best outcomes for the client.
  • Set priorities and monitor workflow in your own area of responsibility including performance indicators from funded programs and Better Together. 
  • Understand and operate computer database requirements to enable the provision of advice, reports, and assistance.

Organisational responsibilities

  • Adhering to established work practices to meet both program and Better Together goals.
  • Support Better Together programs and procedures.
  • Establish and/or be responsible for projects and/or functions to achieve outcomes.
  • Adhere to Records Management policy and procedure by keeping professional case notes and preparing reports according to guidelines.
  • Set priorities, plan, and organise your own work and that of lower-classified staff and/or volunteers and establish the most appropriate operational methods within Better Together guidelines.
  • Collaborate with colleagues, funding agencies and clients while working with in Better Together policies and procedures.

Reporting and Support

  • Provide accurate, succinct, and timely reports on all aspects of your area of responsibility, contributing to team reports when required.
  • Provide back-up and support in the workplace (as required) to maintain services to clients and to the general public.

SELECTION CRITERIA

Qualifications

  • Bachelor’s Degree in Social Work or an equivalent area of study.
  • Driver’s Licence.
  • Capacity to successfully acquire a Blue Card

Knowledge and Skills

  • Knowledge and experience of contemporary principles and practices of social work and case management.
  • Knowledge and understanding of the social issues and barriers around child protection, families, domestic violence, and young people. 
  • Relationship and rapport-building skills. 
  • Willingness to develop a comprehensive knowledge of organisational policies, procedures, practices, and contract requirements with an appreciation of Better Together’s vision, mission, and strategic priorities as well as a detailed knowledge of program activities and work practices relevant to the work area.
  • Cross-cultural understanding.
  • Competent in office software, including databases and Microsoft Office suite of products. 

Demonstrated experience in:

  • Managing priorities and performance in a professional multi-functional team.
  • Communications, both oral and written, including computer office software, spreadsheet, and database use.
  • Case planning, implementation, and management.
  • Client notes and report writing.
  • Engaging a high level of interpersonal skills in dealing with the public and other organisations.
