SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it.An exciting opportunity exists for a reliable and experienced Occupational Therapist interested in working as part of the multidisciplinary Southcare Outreach Service (SOS) providing aged care assessment in the community. The Occupational therapy position plays an integral part in facilitating a quick community service response to ensure safe and effective interventions for clients in the community. The role works within a dynamic and supportive MDT providing the applicant with opportunities to learn about rapid response aged care in the community setting.The Southcare Outreach Service (SOS) is a multidisciplinary community team for the ageing population over 65 years of age residing in the Sutherland Shire. The SOS provides short term acute and sub-acute interventions (up to six weeks, with an average of 2-4 weeks) by Geriatrician, Nursing, Physiotherapy, Occupational Therapy and Social Work to facilitate enhanced patient care and safe clinical outcomes. SOS staff provide assessment, intervention and referral onto other required services in order to navigate and integrate the patient’s journey between tertiary and primary care.The Southcare Outreach Service (SOS) provides a 7 day a week service: public holiday coverage, afternoon shifts (1030 – 1900 hours) will be required in this role.Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified for this position following suitable labour market testing.All NSW Health workers are required to have completed a primary course of a COVID-19 vaccine which has been approved or recognised by the Therapeutics Goods Administration (TGA). Additionally, Category A workers are required to receive a booster dose three months after completing the primary course of COVID-19 vaccinations. New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate (IM011 immunisation medical exemption form) certifying the worker cannot have any approved COVID-19 vaccines available in NSW.Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. - Extensive post graduate clinical experience working in aged care and/or a community setting with the aged population.
- Excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels.
- Demonstrated ability to provide high level clinical advice, education and support to others in providing service excellence.
- Ability to work as part of a multi-disciplinary team and to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel, complex or critical nature
- Demonstrated ability to initiate, lead, complete and share quality improvement initiatives and service evaluation processes.
- Demonstrated active participation in developing, implementing, and reporting on services
- Current Class C Drivers Licence and a willingness to travel in accordance with the demands of the position.
