The objective of this role is to work within a team to ensure the delivery of high-quality clinical care and lifestyle services to enhance the lives of residents.Are you ready to join our team?**Location:

1A The Avenue, Malvern East VIC 3145We are looking for someone who can:

• Develop a balanced therapeutic and social lifestyle program that is responsive to Leisure and Lifestyle at the residence that is based.
• Ensure resident needs are communicated and documented appropriately. This includes having input into resident assessments, person centred care plan development and reviews, and completing 3 monthly ROD reviews.
• Assist in the delivery and/or supervise the delivery of lifestyle programs as per the monthly calendar.
• Ensure all staff follows Occupational Health and Safety policies and procedures.
• Be an excellent communicator who is dedicated to making a difference
