The role of the Primary and Community Health Manager is to provide expert leadership to the Nursing, Allied Health and Administration staff employed within the Ambulatory Care, Renal, Methadone Program and Community Health Teams of the Wellington Health Service.We are seeking a person who is innovative and creative. A person who can design and deliver programs whilst fostering relationships with existing service providers and partners within the community, with a specific focus on the interface between acute and community based care.
