About Silverchain and the Hospital at the Home (HATH) program
The Silverchain WA HATH program was established in 2008 and provides hospital level care for those who are low-risk and require short term acute care that can be delivered safely in the Home and / or a clinic.
About the role
Silverchain are seeking a part-time Medical Practitioner to work as part of a team of doctors and nurses to provide medical care to patients in the WA HATH program. As a Medical Practitioner within HATH, you will be involved in daily team handovers followed by admissions and management of patients including associated administrative duties. Patients will be reviewed face-to-face at the Home, Silverchain clinics or via telehealth, as clinically appropriate. The role provides variety including a mix of community and hospital medicine. Silverchain guidelines and policies are available to guide our clinical staff and you will be supported by a cohesive, experienced team of medical, nursing and administrative staff.
About You:
- General or Specialist AHPRA registration with no conditions
- Medical Practitioners who have 19AB & 19AA restrictions are eligible to apply
- Driving license and own vehicle
- Have a keen interest in community medicine and holistic patient care
- Ability to work effectively within a multidisciplinary team
- Ability to adapt to changes within the service in response to healthcare pressures
- High standard of communication and interpersonal skills to provide clinical advice and support
- Participate in education and professional development of self and others
