AddressMental Health Worker - Your LINC Mount Gambier (Full-time 1.0 FTE and Part-time, 0.8 FTE) Your LINC is a multi-agency, two-tier Mental Health outreach service delivered in partnership by Centacare, KWY Aboriginal Corporation and Aspire Recovery Connection (ARC) a 100% Lived experience organisation. Our service offers holistic, flexible and culturally responsive psycho-social support for individuals experiencing Mental Health challenges and distress across the Adelaide Metro, Barossa, Hills, Fleurieu and Limestone Coast. It is delivered in the Limestone Coast Local Health Network region.The RoleMental Health Workers work with people, and their families/carers where appropriate providing person-centred psychosocial support including goal setting, exit planning, and application and transition to NDIS services.Tier 1 (6 weeks) - Intensive Crisis Support – provision of flexible high intensity case management support.
