About Your Role
Enhancing end of Life funding is to ensure people across NSW have equitable access to high quality end of life and Palliative Care. The Lead Pharmacist - Palliative care is an outcome of this funding and aligns with the End of Life & Palliative Care 5 priority areas and 9 key actions
The vision for Murrumbidgee Local Health District (MLHD) is “Wellness is our goal, Excellence is our passion, Our people are our future”. MLHD is committed to providing the best possible compassionate care when people need it. Plan, coordinate, and implement in-depth levels of clinical expertise and extensive specialised knowledge within the discipline of clinical pharmacy in order to provide high quality end of life and Palliative Care consistent with NSW Health, MLHD policies, procedures and standards.
The Lead Pharmacist – Palliative Care will have responsibility for providing clinical care and care coordination to people across the MLHD who have end of life and Palliative Care needs. This is a new position, suitable for clinicians experienced in Palliative Care and has significant scope for role development iteratively as the service is established and rolled out.
The position will assist with triage of patients to the appropriate community or outpatient pharmacy services and ensure patients have access to required medications. Once accepted to the Palliative Care Service, The Lead Pharmacist – Palliative Care will provide medication therapy assessment, comprehensive medication management, education and support to patients and their families.
Within this role, there will be a requirement to undertake clinical reviews across clinic, community and virtual care settings, either alone or in conjunction with a multidisciplinary team.
The successful candidate will be supported by the District Pharmacy and Palliative Care Teams within MLHD. In addition to the above, there will be opportunities to lead education and research activities and to undertake strategic planning and execution, and capacity building activities for this exciting new service.
