Grand Pacific Health is looking for a Multicultural Care Navigator Officer to support the establishment of the new Head to Health service in Wagga Wagga.

We’re here to provide our local communities in NSW and the ACT with equitable, inclusive and integrated health care including physical health and mental health care services. We currently employ 340 people, provide services from 25 locations and are continuing to grow.
About the position
Head to Health provides high quality, evidence-based assessment, care coordination, information, support, and treatment to people experiencing psychological distress, their family, carers, and support networks. Head to Health offers immediate, short to medium-term episodes of care and service navigation. The multidisciplinary mental health team facilitates a holistic and collaborative approach to care which addresses a broad range of social, physical, and emotional needs.
Wagga Wagga Head to Health is a collaboration between Grand Pacific Health, Directions Health, Riverina Medical and Dental Aboriginal Corporation and Mission Australia.
The Multicultural Care Navigator works collaboratively with consumers, particularly from new and emerging communities to connect consumers with services and supports, facilitating person-centred and integrated care as well as promoting Head to Health services increasing the community’s understanding and awareness.
This position delivers practical support through information sharing and education, identifying and addressing barriers to access and facilitating warm referrals, ensuring consumers are linked to the right service at the right time.
This position is full time opportunity, fixed term until 30 June 2026 with possibility for extension. As a full-time position you have the opportunity to enjoy a rostered day off each month.

We are looking for someone with:
Tertiary qualifications and/or extensive experience in a relevant discipline including mental health, health, community services or social sciences.
Understanding of the needs of new and emerging communities and the ability to work collaboratively with these groups to provide information, care navigation and referral support.
Demonstrated understanding of supports and services available in the local and regional communities
Demonstrated understanding of the health and wellbeing needs of consumers with mental health and/or substance use problems
Well-developed interpersonal, communication and collaboration skills and ability work as part of a team as well as independently
