We are looking for a Health Care Navigator to join our Primary Health and Wellbeing team in Wagga Wagga, NSW. The vacancy is ongoing and will be Full Time, 75 hours per fortnight, but Part Time from 60 hours per fortnight may be considered for the right candidate.
What will I do?
- Provide holistic and coordinated support to individuals with complex social support needs
- Enhance the effectiveness of health-based care coordination activities for participants
- Co-produce personalised support plans aimed at connecting people to local services
- Work in a supportive team as part of a reputable not-for-profit healthcare provider.
What do I need?
A tertiary degree in Social Work or Social Science, Community Services, Human Services, or other health-related discipline, and experience supporting people living with chronic health conditions in a community-based setting.
Additionally, you will have a solid understanding of local community-based services and the ability to advocate and engage these services. You will have an in-depth understanding of the wider determinants of health and utilise this to provide a person-centred approach to participants.
