About the role
In this role you will work as part of the NDIS Team to support clients put their NDIS Plans in place providing Support Coordination. The role has a strong focus on providing comprehensive, coordinated and integrated services to our clients.
Your tasks will include:
- Providing the very best support to clients in line with their personalised NDIS plans and goals
- Co-designing services to meet our clients’ needs including arranging assessments, coordination of services, case management and care planning.
- Working as part of the NDIS Team using a holistic approach to services.

Skills and experience
- Experience/understanding of aged/disability care sector and the ability to manage a client case load in a community care service model.
- Experience in supporting clients to manage their plans as well as developing client focused service delivery plans.
- Demonstrated ability to arrange services to provide safe, appropriate and responsive quality community care.
- Ability to work with a diverse support team and manage client and staff relationships
- Demonstrated ability to engage in professional relationships, excellent communication and interpersonal skills while working within a team in a rural community care setting.
- Good computer and technology skills and as well as the ability to utilise a computerised customer management systems.
- Current National Police Check, NDIS Worker Screening and Working with Children Check will also be required (we arrange and pay at contract offer stage) This is a mandatory prestart requirement.
- Current vaccinations for influenza and COVID-19 are also a requirement of the role
