Your Role
Due to an internal secondment, the team are in need of an NDIS Service Delivery & Intake Officer to assist on a temporary basis for 6-8 weeks.
Reporting to the Operations Manager, your role is to process intake documentation for NDIS consumers, care planning and ongoing Service Delivery. This may include:
- Facilitating the intake process and all documentation in line with NDIS requirements;
- Undertaking intake reviews, assessment and referral processes, including preparing care plans and monitoring ongoing Service Delivery outputs to ensure contracted services are achieved;
- Review consumer NDIS service packages and amend where required;
- Adhere to defined client budgets;
- Liaise with service and support staff and provide assistant with complex cases;
- Provide support to the NDIS Team Leader and Clinical & Support Services Leadership Team.
Your Profile
As this role is for a start on Monday 29th April, you must be available to commence straight away. It is essential you have prior experience in an NDIS Support Coordination or Service Delivery/Intake capacity as well as a current police check and NDIS workers' clearance.
As this is a contract role, you need to be able to quickly uncover processes and procedures and use your initiative to find solutions.
Please note: This role is fulltime onsite, Monday to Friday business hours.
