NDIS Support Coordinator (Part Time - minimum 15 hours per week - with potential for Full Time)

We are looking for a Support Coordinator to join our Newcastle/ Hunter Valley team.

Doing all this while being supported to working autonomously means that they are able to create a work schedule that suits their life.
We aim for excellence in service provision but are not KPI driven. We work with our team members through regular check-in’s to ensure they are supported to remain focused on Participants. Ultimately, we believe in supporting those who support Participants.
Create A Sense of Place focuses on providing completely Independent services so that Participants needs are always at the forefront of our service provision. We are a NDIS registered Organisation, and are certified by the NDIS Quality and Safeguards Commission to provide services throughout Australia so we can work with all Participants who are NDIA managed, self or plan managed.
About the role:
This vacancy is for a part time Level 2 - NDIS Support Coordinator who is available at a minimum of 15 hours a week. Prefer experience but people with transferable skills in case management or in Specialist Support Coordination are welcome to apply.
Our service provision model focuses on ensuring Participants are heard and supported to move towards their goals.
Key functions of this role are:
- Taking a holistic view by working effectively with the Participant to coordinate, implement and maximise on identified NDIS plan goals.
- Build and maintain pro-active relationships with NDIS registered non-registered providers and mainstream community-based resources.
- Ensure Participants are receiving qualitative value for money and encourage self-direction.
- Ensure mainstream services such as housing, health, justice and education meet their obligations.
- Monitoring plan outcomes and expenditure.
- Undertaking regular training, continuous improvement, and staying abreast of key industry changes.
- Deliver clear accurate and timely record keeping.
- Engaging as part of a dynamic team.

You will need:
- Experience as a Support Coordinator or in Case Management
- Access to a working, insured vehicle
- Full Australian working rights
- Valid NDIS worker screening check
- NSW working with children's check
