Position Overview: We are seeking a dedicated and compassionate Support Coordinator to join our team. The Support Coordinator will be responsible for providing comprehensive support coordination services to individuals with disabilities, empowering them to achieve their goals, access community resources, and live fulfilling lives.
Key Responsibilities:
· Conduct comprehensive assessments of clients' needs, strengths, and goals to develop individualized support plans.
· Coordinate services and supports across multiple providers, including healthcare professionals, community organizations, and government agencies.
· Advocate for clients' rights, preferences, and choices to ensure that they receive person-centered care and support.
· Assist clients in accessing and navigating available funding, benefits, and services to maximize their independence and quality of life.
· Monitor clients' progress, evaluate outcomes, and adjust support plans as needed to address changing needs and goals.
· Collaborate with interdisciplinary teams, including family members, caregivers, and allied health professionals, to ensure holistic care and support.
· Maintain accurate and up-to-date documentation, including progress notes, service agreements, and reports, in compliance with regulatory requirements and organizational policies.
Qualifications:
· Previous experience working in a similar role, preferably in disability services or case management.
· Strong knowledge of the disability sector, including NDIS (National Disability Insurance Scheme) policies and processes (preferred).
· Excellent communication, interpersonal, and advocacy skills.
· Ability to work collaboratively as part of a multidisciplinary team.
· Proficiency in using computer software and electronic documentation systems.
· Current driver's license and access to a reliable vehicle (preferred).
