The OpportunityWe're seeking a dedicated and experienced Support Coordinator to join our expanding team in a permanent full-time position.
The RoleCoAbility is an expert in providing exceptional support services to participants with complex needs. As a Support Coordinator, you will manage a caseload of Level 2 and Level 3 Support Coordination clients.
You will support clients in understanding and implementing the funded supports in their NDIS plans and linking them to appropriate services. You will collaborate with clients and their families/representatives to build their capacity to manage the support within their NDIS plan.
You will be responsible for:
- Assisting NDIS participants in implementing NDIS Plans and coordinating their services and supports.
- Completing all necessary administration to the required standard, ensuring full compliance.
- Identifying, developing, coordinating, and facilitating access to supports and services, including coordination of assessments and reports as per the NDIS guidelines.
- Assisting participants and their families in understanding and reviewing their NDIS Plans to ensure they get the maximum benefit and personalised care from their funding.
- Building the capacity of the participants to achieve greater independence to self-direct services and supports in the longer term.
To excel in this position, you will:
- Have a Tertiary qualification in social work, human services, community services or other relevant discipline
- Embrace cultural awareness and inclusivity as integral components of professional practice.
- Have exceptional written, verbal, interpersonal and presentation skills.
- Have a sound understanding of the client experience in the disability and community sector and current best practice principles.
- Deep appreciation for individuals with disabilities and their families, reflecting high ethical and professional standards.
- Comprehensive operational knowledge of the NDIS and relevant legislation, policies, regulations, and service benchmarks.
- Commitment to person-centred and family-centred practices
- Be process-oriented with excellent organisational and time management skills
- Demonstrate a strong work ethic and commitment to excellence
- Possess excellent computer and CRM skills

We are a fully remote team working from home. Travel within Gippsland is required as part of the role.
