About the role
Assist NDIS participants to coordinate and implement a range of support services, in line with their NDIS plan, using a person centred active support approach.
You will establish strong rapport and relationships with a range of stakeholders including People with Disability and/or mental health issues, their family members, NDIS Local Area Coordinators, NDIS Planners, internal and external service providers. This role is permanent full time or part time (30 – 38 hours per week) where you will have the flexibility to work from home and from our South Morang office when required. There will be some travel required to meet with clients across South Morang and surrounding suburbs.
Key responsibilities
- Strengthen and enhance our client's ability to coordinate supports and participate in the community
- Regularly monitor our client's NDIS plan to ensure services are being delivered within their allocated budget
- Ensure mainstream services meet their service obligations to clients (i.e. housing, education, justice, health)
- Provide the client and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequency
Skills & experience
- Demonstrated experience in working with people with a disability and mental health issues
- Excellent verbal communication skills and a demonstrated ability to engage with clients and their families to support them as participants of the NDIS
- Strong written and computer skills for the creation of plans, programs reports, correspondence and other documents
- Established time management, problem solving and conflict management skill
- Ability to think creatively and use initiative
- Unrestricted driver's licence
Successful candidates will be required to clear probity checks including National Criminal History Record Check, NDIS Workers Screening Check and Working with Children Check.
To be considered for and appointed to a position at Life Without Barriers, you must be fully vaccinated against COVID-19 with a minimum of two (2) doses or hold a valid medical exemption certificate, in accordance with Life Without Barriers Employee COVID-19 Vaccination Policy.
