As a Support Coordinator, you will support people with a disability to implement their NDIS plan and link them with other service providers to ensure their support needs are met.
About the role
Empowered’s experienced team of Coordinators offer assistance to individuals and their families on their NDIS journey. You aim to increase the independence of our clients by working closely to develop their capacity, confidence and quality of life. You will support clients to ensure their NDIS funding is utilised appropriately and effectively. You will balance individual NDIS Plan budgets and connect clients with support services in the local area best suited to their needs.
We have a passionate team of Support Coordinators that you will work alongside to achieve great outcomes for the people we support.
Our team of Coordinators pride themselves on professional development and will be able to contribute valuable knowledge as you transition to your new role.
Flexible working arrangements can be an option once full-time face-to-face training has been completed.
About you
We are looking for a self-motivated and enthusiastic person who wants to grow alongside our organisation. You must be well-presented, articulate and fun-loving, whose passion lies within the disability sector, with a desire to better the lives of people living with a disability.
Job Description
- Implementation of all supports necessary, including informal, mainstream, and community supports.
- Assess a number of provider options in the local area.
- Negotiate services and prices as part of any quotable supports
- Connect to mainstream or community services (i.e. housing, education, transport, health).
- Arrange any assessments required to determine the nature of required funding (e.g. Home modifications required, Assistive technology).

Essential Criteria
- Current Drivers Licence.
- NSW Working with Children Check.
- NDIS Worker Screening Check.
- Excellent case note reporting and record management.
- Demonstrated computer skills.
- Knowledge of the NDIS and price guide.
- 6-12 months experience in a related or relevant role.
Desirable Criteria
Qualification in Community Services, Mental Health or related field.
