The Ella Centre, located at Haberfield, is a leading provider of a range of valuable community services to the frail aged, people with dementia and adults with a disability living within Sydney’s Inner West. Services include Disability and Aged Care Group Programs, Individual Support, Social Support, and Support Coordination. The Ella Centre operates from 3 sites all based in Haberfield, providing service users with valued and fulfilling activities and services to enable them to participate in the community, respecting independence.
About the role:
The NDIS Individual Support Coordinator is an exciting 12-month Maternity Leave Cover position responsible for the co-ordination and delivery of a community based social support service for adults with a disability and people with dementia receiving supports under the NDIS.
Role responsibilities:
The role will see you overseeing and coordinating a large team of causal support workers that provide remote person-centred support. You will also be involved in:
- Rostering and allocating staff to various shifts and reviewing timesheets.
- Developing and implementing care plans in consultation with carers and individuals.
- Ensuring quality care and compliance with NDIS and organisational policies.
- Supervising staff and managing budgetary responsibilities.
- Providing culturally sensitive services and accurate program administration
- Promotion of the program within the Inner West Community and maintaining partnerships with community groups.
- Working 7.30am-3.30pm, with weekend on-call responsibilities.
To succeed in the role, you will be a natural communicator with a passion for person-centred care. You will also have:
- Knowledge and understanding of the NDIS, relevant legislation, and budget management.
- Previous experience within the disability field, including working with carers.
- Demonstrated experience supervising or overseeing staff
- Relevant qualifications in social sciences, social work, nursing, disability and/or aged care.
- Strong communication, organisational, and leadership skills.
- Prior experience with rostering for a large pool of staff is highly desirable
- A NDIS Workers Screening Check and a current NSW Driver’s License.
