About the Role
To meet the changing dynamics of General Practice and chronic disease management, we have created a new Nurse Care Coordinator role to support our clinic in driving and monitoring the Coordinated Care program: will include Chronic Disease Management, Health Assessments and ATSI Assessments. The role of the Nurse Care Coordinator is responsible for developing and monitoring systems that support the clinic with delivery of consistent, efficient, and effective care coordination. With a focus on meeting patient healthcare needs and General Practitioner (GP) requirements and ensuring the most appropriate utilisation of the Medicare Benefits Schedule. The Nurse Care Coordinator will be required to work in the clinical setting providing general practicing nursing at a local clinical level.
Commensurate with the role an attractive salary package will be offered.
The Nurse Care Coordinator will:
- Develop and support the implementation of Co-ordinated Care streamlined process for eligible patients and the health care team
- Provide support and direction to Practice Nurses in the area of Co-ordinated Care resulting in efficient and effective delivery of care
- Monitor the performance of clinics and individual nurses in relation to Co-ordinated Care quality, demand, and output
- Assess Co-ordinated Care workforce utilisation and recruitment requirements
- Partner with the Director of Nursing and/or Clinical Nurse Educator (CNE) to facilitate training and education for clinic nurses in expanding their knowledge and competence in their understanding and awareness of Chronic Diseases and providing high-patient quality care
- Seek internal feedback about Co-ordinated Care practices to improve Co-ordinated Care work process and as part of quality improvement activities
- Triage and management of acutely unwell patients
- Immunisations of children and adults
- Assisting with minor procedures, cold chain management, Iron infusions, infection control, ECG’s, and wound care

As an APHRA registered RN/EN, you will bring -
- Demonstrated experience in a Primary Health Care setting or within General Practice
- Excellent written and verbal communication skills
- Strong computer literacy
