Nurse Practitioner Navigator - Rheumatic Heart Disease & Complex Cardiology

About the Role:
As aNurse Practitioner Navigator - Rheumatic Heart Disease & Complex Cardiology, you will provide and coordinate comprehensive care of patients using advanced practice skills in the area of Rheumatic Heart Disease and Complex Cardiology.  The Nurse Practitioner Navigator functions in a collaborative model with the healthcare team to provide assessment, diagnosis, treatment and referral as required.

The Chronic Disease unit aims to deliver evidence-based care to reduce chronic disease burden across the health care continuum, through empowering patients with contemporary education, medication management and primary care engagement. This is done in collaboration with cardiology infection diseases specialists and general practitioners.
