A Nurse Practitioner (NP) is a registered nurse educated and authorised to function autonomously and collaboratively in an advanced and extended clinical role. The NP role includes the assessment and management of patients using nursing knowledge and skills and may include but is not limited to initiation of diagnostic investigations, prescribing of medications and direct referral of patients to other health care professionals. The NP role is grounded in the nursing profession's values, knowledge, theories and practice and provides innovative and flexible health care delivery that complements other health care providers.The Nurse Practitioner is educated and endorsed to function autonomously and collaboratively in an advanced and extended clinical role. The NP role includes assessment and management using nursing knowledge and skills. The role may include, but is not limited to, the direct referral of patients to other health care professionals, prescribing medications and initiating and interpreting diagnostic investigations
Applications are sought from experienced Registered Nurses to join the SLHD Residential Aged Care Facility (RACF) Outreach team. RACF Outreach provides a proactive, comprehensive, holistic, integrated, and specialised aged care service to residents of aged care facilities, in which the quality of care and safety of residents is central. The model is based on delivery of a mix of specialist nursing and medical services to ensure timely access to the most appropriate form of care. The care may be delivered by telephone, face to face or Telehealth. The service operates 7 days per week, from 8am till 8pm, with support from RPAH Virtual overnight.
The Transitional Nurse Practitioner/Nurse Practitioner role provides autonomous care for patients in RACFS to :
- Complement existing SLHD services.
- Ensure patients have access to high quality health care in the community for both acute and sub-acute conditions;
- Reduce hospital admissions, ED presentations and representations;
- Provide nursing assessment, health care management and clinical support at an advanced level to patients and carers;
- Demonstrate a high level of professional practice and clinical leadership that supports and incorporates education and research. This will include continued contribution to teaching outside the clinical environment and strengthening partnerships with the academic sector.
Care is provided in a collaborative multidisciplinary team environment. This incorporates working with SLHD clinicians, as well as General Practitioners, facility staff, patients, and families.
We are looking for someone who has…
- Current registration with AHPRA and authorised to practice as a Nurse Practitioner by the NSW Nurses & Midwifery Registration Board (AHPRA) or working towards such a qualification. If transitional, 5 years full time equivalent post registration experience, with at least 3 years full time equivalent clinical experience in the specialty of the Nurse Practitioner role.
- Excellent organisational, time management and interpersonal communication skills.
- Experience in the use of Telehealth platforms both for clinical assessment and for teaching is desirable. In addition, a high-level proficiency in the use of computer software applications is essential for this position.
- Current NSW Driver Licence (P2 acceptable).

About working for SLHD
This is a Category A position. To be employed by NSW Health you are required to provide evidence of vaccination to comply with the NSW Health Occupational Assessment, Screening & Vaccination against Specified Infectious Diseases Policy (PD2023_022)
All NSW Health workers are required to have completed a primary course (2 doses) of a Therapeutic Goods Administration (TGA) approved or recognised COVID-19 vaccine (except for the Janssen COVID-19 vaccine which is approved by the TGA as a single dose primary course). New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an Australian Immunisation Register (AIR) Immunisation History Statement certifying the worker cannot have any approved COVID-19 vaccines available in NSW. A NSW Health agency may require further information about the medical contraindication (including but not limited to an Immunisation Medical Exemption form – IM011 form).
Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.
