What you will be doing:
You will be adventuring to our clients homes in the community providing assistive technology scope of works, SDA and SIL Assessments or complex home modifications creating tailored therapy plans, facilitating purposeful therapy sessions, and producing NDIS assessment reports.
Responsibilities:
- Conduct NDIS SDA, SIL, and Home modification assessments to develop personalized plans for clients, focusing on enhancing their independence and quality of life.
- Provide therapy interventions and support services to individuals with diverse needs, including but not limited to those under the NDIS programs.
- Collaborate with interdisciplinary teams to ensure comprehensive care and holistic support for clients.
- Offer guidance and education to clients, families, and caregivers on strategies to improve daily functioning and promote autonomy.
- Document client progress, maintain accurate records, and prepare reports as required.
Requirements:
- Minimum 1 year of experience within the NDIS SDA and SIL space and a bonus if you have an understanding of the SDA, SIL, and Home modification assessments.
- Valid Driver's license and reliable car for a probationary period (car provided thereafter).
- Relevant Tertiary Qualification and current AHPRA membership.
- Medicare Provider number or willing to obtain.
- Hold a current Blue Card or willing to obtain.
- Worker Screening Certificate or willing to obtain.
- Yellowcard certification would be advantageous or willing to obtain.
