Occupational Therapist

HammondCare is an independent Christian charity that exists to improve the quality of life for people in need. We specialise in aged and dementia care, palliative care, rehabilitation, mental health services for older people, and other related health and aged care services. HammondCare seeks to embed evidence-based best practices in its services and we provide these services through home care, sub-acute hospitals and residential aged care.

The role of the casual Occupational Therapist is to work within the Mission and Mission in Action of HammondCare to ensure a high standard of Occupational Therapy service is provided to patients of the Hospital, along with appropriate support and advice to their carers. The Occupational Therapist will participate as a member of the multi-disciplinary team to ensure a patient-centred, evidence-based, continuous quality improvement work environment that is consistent with the values and ethics of HammondCare and Occupational Therapy Australia.

• Provision of effective and efficient Occupational Therapy service to clients and appropriate support and advice to their carers
• Participate as a member of the multi-disciplinary team in assessing patient's level of functional and care needs, setting care goals and providing appropriate treatment
• Participate as a member of the HammondCare team in the application of the organisation's Mission and values to the pursuit of its objectives in a client-centred, continuous quality improvement work environment

• A tertiary qualification in Occupational Therapy and registration as an Occupational Therapist with AHPRA
• Significant practical experience or Extensive Knowledge in providing Occupational Therapy Services in a Rehabilitation, Aged Care or Palliative Care setting. Experience across inpatient and outpatient/ community settings would be desirable
• A current driver's licence and safe driving record
• Willingness to travel between HammondCares sites and services
• Effective communication and interpersonal skills
• Demonstrated organisation and time management skills
• Able to work independently and as part of a multidisciplinary team
• Experience in conducting home assessments and equipment prescription
• Empathy and patience
