Occupational Therapist – Full Time

We are looking for an Occupational Therapist to work within a unique service model responding to the current environment of Australian community aged care.

You will be working with our Home Care Provider partners to support their clients to stay in their homes longer while maintaing their autonomy, independence, capacity and dignity.
Clients are supported by their care support staff to implement the programs you develop, multiple times a week across a 12-month period. Allowing for greater frequency of service provision, at a lower client cost than a traditional Clinician-Client approach; and increasing our capacity to provide much needed Allied Health supports to a larger portion the ageing population.
Your Role
- Work with our clients in their homes and via telehealth to develop functional goals.
- Conduct client assessments and reviews and collaborative goal setting.
- Develop individualised programs based on client goals
- Educate and Empower care support workers to understand and support clients to implement the program
- Communicate with Home Care Providers, clients and/or their families across all stages of service delivery.
- Regular assessment & review to monitor progress and outcomes - including desktop reviews to monitor participation and compliance to program instructions.

Mandatory Requirements
- Occupational Therapy Degree
- Current AHPRA registration (including Professional Indemnity)
- Current Influenza vaccination
- Covid-19 Vaccinated
- NDIS Worker Screening
- Unrestricted QLD Drivers Licence
- Experience working with in Community-based Aged Care preferred
