• Based in WA’s Midwest region, enjoying amazing beaches and relaxing lifestyle
• Engage with aged care clients eligible for CHSP funding in the comfort of their homes
• Conduct personalised assessments focusing on customers' functional abilities, strengths, and lifestyle aspirations
• Collaboratively develop individualised therapy plans in partnership with customers and their caregivers
• Offer clinical guidance and recommendation for assistive technology and home modifications
• Prepare reports and manage funding applications
• Coordinate referrals and liaise with stakeholders on service planning, delivery, and evaluation

About you:

• Experience as an Occupational Therapist with current AHPRA registration
• A strong understanding of assistive technology and home modifications
• Proficient skills in report writing
• Superb interpersonal and communication skills with the ability to build rapport with and engage clients and families
• Self driven, confident, patient, creative with a client centred approach
