Practice Information:
This is a young and growing medical practice with a strong focus on internal multidisciplinary team integration and external collaboration with like-minded providers. They aim to provide exceptional primary healthcare to the community in a safe, culturally appropriate environment. Their patient-focused approach includes outreach services and mobile clinics, promoting good health and reducing inequalities. They prioritise health promotion and prevention and empower our patients to make long term, sustainable changes. The practice specialises in Indigenous health, mental wellbeing, disability supports, advance care planning, LGBTIQ+ and family planning. They offer a range of billing options including Medicare, DVA, NDIS, mixed and private billing. They focus on quality, safety, performance, and accountability in a warm and welcoming environment. The practice offers a supportive work culture and focus on integrated multidisciplinary support, including weekly case conferencing for complex clients/patients.

About the Role:
This practice is looking for a qualified Occupational Therapist to join their team and make a positive impact on the community. As an Occupational Therapist, you will be responsible for conducting assessments, developing treatment plans, providing intervention services, and managing a complex caseload independently. You will also collaborate with other healthcare professionals to provide holistic care to our clients.

Responsibilities:

  • Conduct assessments to determine the client's functional abilities, strengths, and goals.

  • Develop individualised treatment plans that focus on improving the client's daily living skills and enhancing their participation in social, recreational, and vocational activities.

  • Assess, plan, implement, and evaluate treatment plans to increase patients' productivity and self-care.

  • Provide intervention services to help clients achieve their treatment goals, such as improving fine motor skills, increasing mobility, and addressing sensory processing challenges.

  • Use verbal and non-verbal communication methods to address the needs of patients with communication difficulties.

  • Support the development of discharge and contingency plans to arrange on-going care in various settings.

  • Review, evaluate, and change rehabilitation programmes to rebuild lost skills and restore confidence.

  • Provide advice on the selection and use of assistive technology and adaptive equipment to promote independence and participation.

  • Help patients to adapt to and manage physical and mental health long-term conditions through the teaching of coping strategies.

  • Develop and implement a seamless occupational therapy support service across the practice to improve patient care.

  • Manage a highly complex and specialist caseload independently, demonstrating autonomous clinical judgments where expert opinion differs.

  • Design patient and carer information, including advice to support management of conditions.

  • Deliver best practice in clinical practice, caseload management, education, research, and audit to achieve practice and local population objectives.

  • Collaborate with other healthcare professionals, such as physiotherapists, speech therapists, and psychologists, to provide a holistic approach to client care.

  • Provide education and support to clients, their families, and caregivers about strategies to promote independence and manage their condition.

  • Conduct home and workplace assessments to identify and address environmental barriers that may impact the client's ability to participate in daily activities.

  • Advocate for clients to ensure they receive the appropriate services, support, and funding to achieve their goals and maintain their independence.

Qualifications & Experience:

  • Bachelor's or Master's degree in Occupational Therapy from a recognised institution.

  • Current registration with the Occupational Therapy Board of Australia.

  • Eligibility for membership with the Australian Association of Occupational Therapists.

  • Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans.

  • Experience in risk assessment and management.

  • Experience of assessment and treatment delivery in long term conditions and active management.

  • Experience working in a multidisciplinary team environment.

  • Working knowledge of Microsoft and GP practice and prescribing data monitoring systems.

  • Ability to write comprehensive clinical notes, implement and evaluate care plans.

  • Demonstrated knowledge of and compliance with all relevant legislation and common law obligations.

  • Knowledge of how to identify and respond to unsafe practice.

  • Strong written and verbal communication skills.

  • Ability to work under pressure and meet deadlines.

  • Excellent organisational and time management skills.

  • Ability to work effectively with a diverse patient population.

Salary Information:  $70,000 - $100,000 per annum
