About the Role:
The Regional Model of Palliative Care Navigation forms part of the South Australian Palliative Care Navigation Pilot. The Regional Palliative Care Navigation service will provide information, service linkages, and advocacy to individuals with life-limiting illnesses, their families, caregivers, and healthcare professionals seeking support across the Yorke and Northern Local Health Network.
Services will be delivered via a combination of both telehealth and face-to-face services.
This project is supported by funding from the Australian Government Department for Health and Aged Care and the support from the South Australian Department for Health and Wellbeing
Experience Requirements:
• Demonstrated experience working within a fast-paced environment.
• Proven successful delivery of Care Navigation/Coordination services or other relevant palliative care experience.
Qualification and Other Requirements:
• Relevant tertiary qualification in health/allied health
• Current Working With Children Check
• Current Child Safe Environments
