Aged Care Opportunity in Fleurieu Peninsula
Aged Care Navigators work to connect clients and their carers with services in their area. Navigators work with clients to understand their needs, link to My Aged Care and other schemes, and to match with services once funding is in place. Candidates will work closely with the team but also require some degree of autonomy.

About the role
Use your knowledge and customer service skills to build relationships with people navigating the Aged Care sector and assist them to find the services they need to maintain their independence and quality of life.
About you
You are a great communicator and enjoy working with clients in their community, supporting them to live their lives in the way they desire.
- Relevant qualification in Social Work, Aged Care or Human Services and/or demonstrated professional experience working with clients to deliver a community-based service within the Aged Care sector.
- Excellent written and verbal communication skills
- Proficient in Microsoft Office and client software programs
