The Peer Supported Transfer of Care (Peer-STOC) initiative will provide assertive support and post discharge follow-up for consumers who are transitioning between acute Mental Health care and the community.
The Peer-STOC initiative will work to actively reduce the 28-day readmission rate for consumers leaving acute Mental Health care by providing peer support for up to 6 weeks following discharge.
The Peer-STOC initiative offers hope to consumers who are leaving acute Mental Health care, facilitates contact with people who have a personal lived experience of recovery and firsthand experience in wellbeing management.
Peer-STOC Peer Workers will provide assertive in-reach for consumers in acute Mental Health units in the 7 days prior to their discharge. These meetings are to establish relationships, build rapport, discuss wellbeing strategies and collaboratively identify supports needed to transition into the community.
Peer-STOC Peer Workers will provide direct face-to-face follow up on the day directly following discharge, and no later than 72 hours after discharge. Support will be provided for up to 6 weeks following an acute admission.
Concerns over consumer wellbeing will be escalated to clinical staff within the Community Mental Health team they are located in for assertive follow up to support hospital avoidance.
Priority for referrals should be given to consumers who do not have established connections with a GP or other primary care clinicians, and who are not connected to any other Mental Health service providers.
