About you:
You are a qualified Pharmacist with experience in a clinical pharmacy position and a demonstrated ability to apply evidence in a practical manner to achieve improved patient outcomes. You have demonstrated high-level interpersonal and communication skills including the ability to consult, negotiate and maintain relationships with senior health professionals and other internal and external stakeholders and ability to work co-operatively with less experienced pharmacists and pharmacy assistants to deliver clinical pharmacy services while fostering teamwork and professional development.
Your opportunity:
To deliver high-level clinical pharmacy services within the Community Services as a member of a multidisciplinary team with a focus on First Nations and Rural and Remote patients across the Region.
To provide care and education using Telehealth and internet services to rural and remote regions to improve equity of access for patients' self-identifying as First Nations.
To provide a specialist Pharmacy resource in First Nations Care delivering high-level pharmacy services to a complex and varied patient cohort within CHHHS and Torres and Cape, with a high degree of independence and initiative and to develop services to promote and maintain best practice care of patients.
Your role:
- Deliver high-level clinical pharmacy services within the Community Care team within the CHHHS in accordance with prescribed professional and ethical standards with infrequent need for direct clinical practice supervision.
- Conduct Telehealth medication review with patients requiring Community Care in rural and remote areas.
- Apply professional judgement to a complex caseload, adapting routine clinical practice based on valid and reliable evidence.
- Monitor clinical work practices and develop innovative and novel systems to assist all members of the Community Services through the development of information resources, which includes legislation, pharmaceutical information, drug therapy protocols and relevant published resources.
- Participate in a structured mentoring program for pharmacists including community providers to develop specialist skills within the rural and community service for patients' self-identifying as First Nations and for Rural and Remote patients.
