Job location
Thomas Embling Hospital, located in Fairfield just 15 minutes north of the Melbourne city centre. Due to our announced expansion, we have a need for experienced like-minded professionals to join our team. Thomas Embling Hospital is a secure Mental Health, currently 134 bed, hospital providing acute care, rehabilitation, and continuing care programs. With planned expansion commencing 2024 with significant increase in access to care, we will have a dedicated Women's service and an additional medium secure men's facility. About the Role:
The Physical Health Care Nurse will work closely with Unit based clinicians to ensure the effective delivery of Health Care interventions on an ongoing basis. The Physical Health Care Nurse will work collaboratively with the General Practitioners and visiting clinical allied health specialists to ensure timely and coordinated access to Health Care providers who are linked with the different parts of the health system, allied health providers, pharmacy, pathology services, specialists, or hospitals
Duties:
- Receives patient referrals from Unit based clinicians for patients with acute and chronic health problems.
- Documentation of outcomes of referrals and other Physical Health related assessments and screening processes in PMI.
- Assists clinical teams perform Physical Health assessments and relevant screening processes and monitoring for patients with complex chronic health problems e.g. diabetes, cardiac disease, respiratory disease, cancer, BBVs.
- Assist clinical teams in monitoring patient sexual health needs
About You:
- Demonstrated experience working within a mental health setting
- Demonstrated commitment to mental health consumer recovery.
- Extensive knowledge and demonstrated experience in current primary health nursing practice within a relevant setting.
- Demonstrated ability to undertake comprehensive Physical Health assessments and to plan, implement and evaluate Physical Health Care interventions.
- Proven ability to plan, implement and evaluate educational programs in a Health Care setting.
What you need:
- Postgraduate qualification in Mental Health Nursing or equivalent.
- Registration with the Australian Health Practitioner Regulations Agency (AHPRA).
- The successful candidate will be required to register their qualifications on eCredential
