Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in The Auburn, Blacktown, and The Hills Shire, Holroyd and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region. What you'll be doing
The Hospital in the Home (HITH) Physiotherapist (PT) is responsible for the provision of high quality, patient centred, evidence informed physiotherapy services that promote identified patient health outcomes. The PT will assess, plan, implement and evaluate the physiotherapy care plan in collaboration with individuals and the multidisciplinary HITH healthcare Service.
The PT will provide clinical services to HITH patients, including those with complex care needs requiring advanced practice skills and the application of professional knowledge and judgement.
The PT will exercise in-depth professional judgement when required in resolving problems and managing cases where principles, procedures, techniques and methods require expansion, adaptation and /or modification.
The PT provides clinical supervision for less experienced staff, and contributes to the planning, implementation, evaluation and reporting of HITH healthcare services to meet required Accreditation Standards and WSLHD Executive Strategic Plan.
